



How to complete the 409A request form
The information provided on the previous pages will automatically populate within Carta’s 409A request form. Once the form has been generated, the user will be directed to complete the remaining fields in order to submit the valuation request.

Request details
Verify the status of the company’s most recent 409A valuation, select the desired Valuation date, and check the box to confirm that no securities have been issued since that date. Click Next to continue.
Indicate whether the company has had an external valuation that is not reflected on Carta, and provide additional information about the 409A.

Company overview
Ensure that all fields marked with an asterisk contain up-to-date information. When finished, click Next.
Select the company’s CEO from the dropdown, and indicate a Point of contact for 409A who will provide information about the company on the 409A kickoff call.
Indicate the company’s Audit status, select the company’s Law firm from the dropdown menu, select or attach the Incorporation document, and optionally, attach a copy of the Board / pitch deck.

Description and updates
Provide a Business description, indicate company performance and describe any Company updates since the last 409A (or in the last 12 months), and provide three keywords or phrases that best describe the company’s product/services. Click Next.
