To ensure a smooth onboarding process, please complete all three tabs of the spreadsheet with the required information. A full breakdown of your holdings is necessary for proper account setup.

Does the name have to be the legal name of the company?
Yes, please use the company’s legal name.
Can I use the name that we use internally?
No, it’s best to provide the legal name of the company.
Are these only active companies, or can I put in exited companies as well?
Portfolio Monitoring is designed for tracking active companies. Please enter only active companies and active positions.

Do I need to provide details for investments already tracked in Carta?
Yes—this ensures that your ownership records match Carta’s system of record. It also helps our team ensure that all securities and ownership details are accurately accounted for.
We converted shares from Convertible Notes to Preferred Stock. Should we include the full history?
No, you do not need to provide the full history. Only enter where your holdings stand today.
Portfolio Monitoring tracks ownership from the moment your portfolio is onboarded into Carta, so previous conversions aren’t required.

What is the “Note Block Name”?
The Note Block Name is any name assigned to the security—for example:
"Convertible Note"
"CN-1"
"Convertible Note 2018"
"SAFE"
This helps with accurate tracking of securities.
My SAFEs don’t have maturity dates. What should I enter in “Maturity Date”?
Enter “n/a” or a similar placeholder.
Is the Price Cap entered as per-share or total?
The total, not per share.
What is the “Discount Percent”?
The discount applied to the next round (typically between 0% – 30%, with 20% being the most common).
